Faith Formation Online Registration
- Faith Formation Classes for 2024-2025 are in progress:
  
  Directions for registering (May 15, 2024 was the deadline for Early Bird Sign Up for the new year; all registrations after will need to pay the $25 late fee unless you are a new family joining the parish/program):
  Let's get started registering for 2024-25!
  1.  Complete Section 1 below for students entering First Grade in the Fall of 2024 through 11th Grade.
  2. Complete Section 2 below which asks for parental permissions, t-shirt information, and etc.
  3.  Visit our online Payment Portal to pay for tuition by clicking on the button in the Payment section below. If you have credits, please figure your amount due from the tuition table below and enter that amount on the OTHER line in the payment portal. If your total is zero, you do not need to complete the payment section, just Section 1 and 2.
  
  Some common questions and things to note:
  1. Don't forget to complete the sacramental fees if your child will be a 2nd grader or Junior in 2024-2025. We also need copies of Baptismal Certificates for these students.
  2. Your Junior will be receiving Confirmation as a Senior in the fall. Once you have paid the Sacramental Fee due as a Junior, you do not need to pay or register when your student is a Senior.
  3. If your payment amount differs from the preset amounts (scrip credit, catechist credit), use the OTHER tab to enter that amount (determine your amount due and enter this amount in the OTHER line. If amount due is zero, you do not need to do the payment part, only Section 1 and 3).
  4. Please share our registration with your friends and family who may not be a part of a religious education program.
  5. If you are registering your Notre Dame Junior for Confirmation, please use the paper form below and return to the Parish Office with payment (you may pay the fees online in the Sacrament area for Confirmation).
  Scrip Information and a paper Registration Forms are available below if you wish to complete registration this way along with a check. Please return your information and payment to the Parish Office at 1307 Lourdes Ave, De Pere, WI 54115 (by May 15 to receive the early bird discount).

- We are proud of Our Lady of Lourdes Faith Formation Program and the role that it plays in developing the faith life of our youth. Our program strives to give our children the Christian values they will need to face the challenges in today’s society. The faith development of our youth is a team effort. It involves the pastor, religious education coordinator, catechists, Faith Formation Committee and the family. We ask God’s blessings on all of us as we work together and support each other in this ministry.
  
  At all levels, there is a strong emphasis on scripture and prayer besides the basics of the Catholic tradition. Music, films, videos and a variety of activities enhance the program texts. Students’ learning abilities at each level are considered and a full curriculum is set up to ensure that student will cover a variety of topics over the years of their religious education. Lessons emphasize a message about faith and teach children to apply that message to everyday life so that the religious teachings become a lived response to God in daily actions.
